The Reuse Form is used when there is an existing system available and it meets the requirements of Chapter 72.22. Permit Issuance(h)... "A permit is not required when a new dwelling is proposed to replace a previously existing dwelling when the local agency determines that the size and anticipated use of the new dwelling, as determined under § § 73.16 and 73.17 (relating to requirements for absorption areas; and sewage flows), are the same as or less than those of the previously existing dwelling and the previously existing dwelling was in use within 1 year of the anticipated date of completion of construction of the new dwelling. This exception does not apply when an active investigation of a malfunction is under way by the local agency or the Department." This means the existing drainfield can be used and if you are making any alterations to the building sewer connection, upgrading your tank capacity and or the drainfield a permit is required.

The Complaint Form is a written form that must be kept on file by this office when a complaint is made. A complaint is a serious matter that must be followed through in accordance with the PA Sewage Facilities Act 537, the Rules & Regulations of the PA Code Title 25 and the Policies and Procedures of the corporation. The complainant is kept anonymous and all complaints are investigated, once the investigation begins the complaint is no longer part of the investigation and does not play an active role.

The Holding Tank Pumping Contract is used anytime a permit for the installation of a holding tank is issued for any reason, repair, interim use, low flow or high volume. The is a legal agreement between the Owner(s), the Municipality, the Pumper and the Dispsoal Site. The agreement binds the Owner(s) and future owner(s) to maintain the holding tank(s) in accordance with the terms of the agreement.
